Company Description
Caravan Health is the market leading provider of value-based care (VBC) enablement services to hospitals and physician practices, enabling providers to qualify, participate and succeed under value-based payment models, with a focus on Medicare Accountable Care Organizations (ACO). Caravan drives change across clients’ operations via a collaborative model that allows clients to learn from each other, all enabled by Caravan’s practice improvement and data analytics teams that guide each client through ongoing improvement initiatives.  Furthermore, nationwide workshops train clinicians and practice managers on population health and VBC competencies that attendees bring back to their practices to improve patient care and ultimately lower costs.  The shift to VBC requires providers to have a fundamentally different set of capabilities in order to succeed in these complex programs, driving a significant need for Caravan’s capabilities and expertise. Caravan’s platform drives industry leading savings, care improvement metrics, and strong customer ROI. The Company’s ACO clients achieved more than $26M in savings in 2016, with average savings per patient more than five times the national average.
